Job Description
Position Overview
The Position of Nurse will be based in UHI priority HFs of Herat city and will work to provide patient
care at OPD, IPD, operation theatre and Emergency units of the HFs
Position Reports To: Cluster Manager,

• Location: Herat
Responsibilities
• Maintaining accurate, complete health care records and reports.
• Administering medications to patients and monitoring them for side effects and reactions.
• Prescribing assistive medical devices and related treatments.
• Recording patient vital signs and medical information.
• Screen all
• Monitoring, reporting, and recording symptoms or changes in patient conditions.
• Administering non-intravenous medications.
• Assessing, implementing, planning, or evaluating patient nursing care plans by working with
healthcare team members.
• Modifying patient health treatment plans as indicated by patient conditions and responses
• Regulation and administration of TB drugs
• Establishing a system to evaluate and review TB treatment (form TB 03)
• Provide necessary recommendations to TB patients regarding how to use anti TB drugs, side
effects of anti TB drugs.
• Preparing a recording and reporting system
• Implementing of DOTS at health facility level
• Using treatment cards (forms TB 01 and TB 02) during the treatment
• Weighing patients every month
• If the patient does not come to the clinic within 48 hours of his last visit, he should
follow up the patient and visit the patient at home.
• Collaborating with the doctor and under the guidance of the National Tuberculosis Control
Program, applying for sputum sample
• When the patient is transferred from another health facility, the treatment result report
must be sent to the main health facility.
• Research about the previous TB treatment of the patient
• Contact screening of people who are in contact with tuberculosis patients.
• Maintenance of patient documents
