Job Description
Overview:
As a Nurse for our DICs, you will play a critical role in providing healthcare services and support to individuals accessing the DICs for assistance with substance abuse issues. Working closely with the project team, you will deliver high-quality nursing care, health education, and harm reduction interventions to promote the well-being and safety of DIC clients.
Key Responsibilities:
- Health Assessment and Monitoring:
- Conduct comprehensive health assessments of DIC clients, including physical examinations, vital signs monitoring, and medical history reviews.
- Assess clients for substance abuse-related health issues, infectious diseases, and other medical concerns, identifying appropriate interventions and referrals as needed.
- Healthcare Services Delivery:
- Provide nursing care and treatment to DIC clients, including wound care, medication administration, and health counseling.
- Administer vaccinations, screenings, and diagnostic tests as appropriate, following established protocols and guidelines.
- Collaborate with healthcare providers and community resources to ensure continuity of care and access to additional medical services for DIC clients.
- Harm Reduction and Education:
- Educate DIC clients about harm reduction strategies, safer substance use practices, and preventive health measures to reduce the risk of injury, illness, and transmission of infectious diseases
- Distribute harm reduction supplies, such as sterile syringes, condoms, and naloxone kits, and provide training on their proper use and disposal.
- Facilitate health education workshops and group sessions on topics related to substance abuse, infectious diseases, and overall wellness.
- Collaboration and Referral:
- Work collaboratively with other members of the DIC team, including peer workers, counselors, and outreach workers, to provide holistic care and support to clients.
- Refer clients to external healthcare providers, specialty services, and community resources for further assessment, treatment, and support as appropriate.
- Documentation and Reporting:
- Maintain accurate and up-to-date medical records, documentation, and progress notes for DIC clients in accordance with organizational policies and regulatory requirements.
- Prepare reports and summaries of nursing interventions, client outcomes, and program activities to support monitoring, evaluation, and quality improvement efforts.
Job Requirement
Qualifications:
- Bachelor's degree in nursing (BSN) from an accredited institution required; Registered Nurse (RN) licensure or equivalent certification in the relevant jurisdiction.
- Minimum of two years of clinical nursing experience, preferably in a community health, public health, or substance abuse treatment setting
- Strong knowledge of primary care principles, preventive health practices, and harm reduction strategies related to substance abuse
- Proficiency in medical documentation and electronic health record (EHR) systems.
Effective communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team environment.
