Job Description
M&E is the person with high qualification of MD or MPH and will carry out services like supervision, monitoring for improvement quality of care in the functional health facilities (HFs). He/She will regularly supervise, Monitor HFs activities and will submit progression report to the respective origin. He /she must have full familiarity in BPHS Package implementation.
Job Requirement
- Regular supervision and monitoring of HFs of their respective provinces.
- Providing mission reports, action plan for gaps improvement and sharing systemically with regional Health Program Manager.
- Weekly HMIS reports submission (Each Monday of the weakened) with HMIS officer.
- Bi-Weekly nutrition reports providing and submission to HMIS officer regularly without any discontinuation.
- Monthly MAIR reports collection from HFs and submission to HMIS officer for data entry in DHIS2 data base.
- Orientation to HFs health workers in each supervision and monitoring.
- Monthly action plan providing and signing through RHPM for smoothly implementation.
- Submission of monthly reports at the end of each month to RHPM to measure the health services running status and quality.
- Strictly follow up progression of MoPH feedback action plan for gaps correction.
- Participation in monthly PPHCC meetings, Nutrition meeting, Adhoc, meetings and so on without apologizing.
- Quarterly FSR report submission to HMIS officer from all HFs.
- Maintaining good coordination and communication with PPHDs, PPHOs and other stakeholders such as WHO, MoPH and community.
- Providing and assuring HMIS tools, medicines and medical equipment's in the HFs for effective implementation of medical care services.
- Participation in Joint monitoring with PPHDs, PPHOs, WHO, MoPH and so on.
- On time response to all out breaks and reporting on timely manner for best coordination.
- Verify leave of health facilities staff to avoid from continued services interruption.
- Try to solve all the gaps on the spot in the HFs to ensure quality of care.
- Regularly follow up of the EPI services in the HFs along with all the basic BPI-IS components.
- Deliver all health relevant activities assign through his/her line manager.
- Enter all HMIS data in DHIS2/NND…
- Any other official task assigned by line supervisor and head quarter team.
