I-GENERAL Responsibilities:

The overall scope of work for the Field Monitor/ Supervisor is planning, implementing, supervising, monitoring, data verification/cross-checking, and following up on health programs and activities service delivery of MHT in Herat provinces in line with the MHT standards and work plan of the project.

II-SPECIFIC DUTIES:

The project Field Monitor/ MHT Supervisor is responsible for accurate, consistent, and reliable recording and reporting system in the relevant HFs. S/He will monitor the MHT during the week to:

1. To conduct the supportive supervision according to the plan from MHT on weekly bases at the district level.
2. At the end of the month, s/he will check all the registers with the MAIR of the HFs and ensure that they match with each other and there is no any discrepancy between registers and the MAIR.
3. During the month, s/he will conduct CAF prepared checklist in the MHT and submit the filled checklist to the management of sub office for follow up.
4. In addition to the above-mentioned tasks, s/he will monitor/supervise the MHT and perform other tasks as per his monthly action plan, job description and direction of his/her line manager.

A. M&E

1. Develop and plan a detailed M&E work plan for health facilities based on the project activities. Leading the target setting for key project proxy outcome indicators at the MHT facility level.
2.  Apply standard National HMIS in all MHT facilities under the project
3. Monitor the HMIS indicators at the HFs as well as overall at the project level.
4. Provide support to the standard HMIS database and NMC database and timely HMIS reporting and replica to PPHD.
5. Provide reliable, accurate, and timely information and reports for informed planning and decision making at the project level, and Provide feedback, prepare action plans and follow up.

B. Quality of MHT Services:

Implement the quality assurance/improvement initiatives and approaches (NMC, IP, waste management, etc.) in the assigned MHT including all phases (planning, implementation, and evaluation)

C. Supervision and Monitoring:

1. Support the project management team in adopting a supportive supervision approach in MHT
2. Develop a detailed supervisory action plan at the beginning of each month for the assigned area and submit the timely monthly report to the line Manager.
3. Conduct regular visits on a monthly base to the MHT and keep the remaining days for the administrative issues
4. Cover all sections and staff categories of the MHT in providing support and on-the-job training during supervisory visits
5. Use specific checklists developed for supervision purposes
6. Identify gaps, areas to be improved, challenges, and obstacles in service delivery, find appropriate solutions and address the gaps and problems accordingly.
7. Seek support, where needed, from other staff, supervisors, and stakeholders who can contribute to the solution of the identified problems, gaps, and challenges
8. Provide HFs staff with appropriate feedback and take on-the-spot actions where possible and within the jurisdiction of his/her authority
9. Implement and follow up the agreed actions till the identified gaps are filled and challenges are overcome
10. Conduct meetings with the MHT staff at the end of each supervisory visit and share all points to be improved.

 D. Recording and Reporting:

1. Ensure all the activities under this scope of work mentioned under A to D are properly documented.
2. Ensue that project-related data is collected, analyzed, and used for further improvement of MHT services and management.
3. Support the project coordinator in developing Monthly Updates and reports
4. Support the project coordinator in preparing Technical reports of the project based on MoPH or donor standard format
